2TRAINING SESSION FOR POLICY IMPLEMENTATIONAbstractThis training session will break down two new strategies that Mercy Medical Center will implement to decrease the rate of readmission for our heart failure patients. Our nursing staff has the greatest involvement with hospitalized patients and will be trained to use our new system andguidelines during a two-hour workshop. The workshop lessons involve utilization of computers and training guides so that our staff can practice and apply their new skills. The first part of the workshop focuses on the electronic nursing documentation system and the second part of the workshop will be lessons on each part of comprehensive discharge planning.
3TRAINING SESSION FOR POLICY IMPLEMENTATIONCollaboration & StrategiesTwo new strategies will be implemented within the new practices: Electronic nursing documentation (END) and comprehensive discharge planning. END is found to improve the carequality for patients by decreasing errors in documentation and reducing time documenting (McCarthy, Fitzgerald, O’Shea, Condon, Hartnett-Collins, Clancy, Sheehy, Denieffe, Bergin, & Savage, 2018). This system will highlight what must be documented to ensure detailed accurate notes aided by visual cues and a real time feedback system. Transparency and additional metrics will aid in decision-making (Brown, 2015). Comprehensive discharge planning reduces hospital readmissions and ensure enhanced patient care (Besler, n.d.). According to Knight (2015), discharge planning’s successful transitioning can reduce the likelihood of a 30-day readmission. This plan will include management of symptoms, discharge planning, discussion of medication, and post discharge follow up. Nursing staff is directly involved with the admission and discharge process as well as anything that happens between those times. During documentation, nurses are the last staff members that can correct errors. It is important to train nursing staff on policy guidelines, especially inexperienced nurses that are not able to anticipate errors. For successful implementation of electronic documentation and comprehensive discharge planning, training on how to properly use these processes are crucial. To encourage collaboration, an open communication environment will be fostered to analyze areas of necessary training that the employee needs to improve their performance (Healey, 2018).
