Models and Barriers to ChangeWorking in a critical setting, the primary reason for documentation of code blue is to provide records of interventions occurred during the resuscitation process. Once clarified, the verified information can assist in providing suitable feedback & education to unit/staff/leadershipand code blue teams. In this regard, the biggest barrier identified in my institution is that there is no opportunity to address documentation issues in a timely manner, leading to a lack of knowledge regarding best clinical practices and registry measures to be implemented. The staff iscontinuing the same practices hence making the same errors over the period of time.To overcome this barrier, the current model utilized is part of the Institute for Healthcare
