UBUS 311 Peer Participation Evaluation Form #1 Cross-Functional Team Meetings The Peer Participation Evaluation Form is used by students to evaluate each team member’s contributions to the team effort for the Cross Functional Team Meetings. Scores will be compiled and totaled for each student and an average score determined. Step One: (Required) Evaluate the performance of each member of your team using the criteria below. List all members, including yourself, alphabetically by last name and mark scores in the appropriate boxes. Add the scores across to compute the Total Score. Maximum number of points is 20. 1. Analysis and Creativity. Intellectual involvement in team problem solving on projects and activities, including in-class team exercises and discussions, team meetings and projects. Performing research, formulating ideas, studying decisions, developing communications, etc. Not very involved 0 1 2 3 4 Very involved 2. Contribution. Participation in team meetings, team projects and activities. Low level of contribution
