Behavior Change Portfolio HSPTS 132 Spring 2010 An important part of Personal Health is the knowledge of healthy behaviors. However knowing how to make healthy changes can be even more important. Behavior change tools are crucial in being successful and teaching others how to take on healthy behaviors. You will provide a folder that will be turned in throughout the semester with the following items. See me with any questions before the due dates listed. PART 1: DUE Thu 1/28 (50 points) Health Appraisal Surveys – provide a copy of your results and the typed summary. 1. Surveys: Complete the following surveys a. How Healthy Are You b. Nutrition c. Fitness d. Relationships e. Alcohol or other drug abuse f. Stress g. Disease Prevention h. Environmental Health 2. Type up a summary (1 paragraph) for each assessment. You can put all summary paragraphs in one document to turn in. Include the following: a. What were the results of your assessment? b. Were you surprised or did you expect these results?
