Records management would have to make certain every patient were asked these questions and all the necessary forms were signed during the hospital registration process. Initially it was undoubtedly difficult to ensure every office affiliated with the patient self determination act. When the patient is doing the paper work they have to by law since 1990 sign a paper that states if they have a living will or not and if not if they would like to have one. I think that when you are dealing with the medical records you have to be very careful in what you look at and what you say to the patient or to anyone for that matter. I think that the patients privacy is very important the only people that should know about the patient is the doctor and the insurance company.
