Continued: Pernicious & Fe Deficient Anemia Pernicious s/s- sore tongue, numbness hand/feet, neuralgia (nerve damage), weakness, chg. . LOC, confusion, Elderly Population risk: Dementia vs. Vit-B-12 def. Vs. Alzheimer. Vit B-12 needed for neurological activity ( Iron)Fe- Deficient Iron supplements oral- liquid-use straw-stain teeth ; s/e- constipation/diarrhea nausea/black stools IM- route Imferon-Z- track so won’t stain skin.

General s/s of Anemia Fatigue, weakness, dizziness, pale, blurred vision, irritability, general malaise, increase in resp. rate (tachypnea) or shortness of breath (SOB), tachycardia. In addition clients with Pernicious anemia will have sore tongue, numbness/hands/feet because Vit-B-12 necessary for normal neurological activity and function!

Aplastic Bone Marrow replaced with Fatty material and unable to produce RBC, platelets, WBC, “Pancytopenia” S/S bleeding,bruising petechiae, infection, SOB Bleeding/Risk for Infection Precautions Macrolytic Caused by Folic Acid or Vit-B-12 deficiency. Both necessary for normal RBC production (heme)-iron containing part of RBC Many drugs can cause this problem and interfere with normal absorption Don’t overcook foods Folic Acid/Folate daily

Sickle Cell Anemia -inherited Dz. RBC’s- sickle shape not spherical RBC-easily broken & cellular contents leak out into circulation and aggregate/clump up into *joints-elbows/knees primarily also bones/organs/ spleen too Any time a dec. in oxygen tension is sensed, the RBC begin an observable change process from spherical shape to a sickle or crescent shape which are very rigid and easily cracked and broken… congestion, clotting, clumping
