GlossaryClient– a person receiving a service in their home.Worker– personal carer, care provider, nurse, social/welfare worker, therapist or other people performing health care or community work at the direction of an employerPrimary carer-person who otherwise provides care to the client.Workplace– any place where work is, or is to be performed by a workerCare/case manager- Someone who provides individually planned and coordinated, flexible packages of care to provide for the complex care needs of older peopleService delivery plan-A plan used by services to detail the use of funding assigned to support a person. Service delivery model in community settingPeople can live in their own home or a rented house/unit in the community. Care worker assist with the support they required.People can live in supported accommodation units. A few people with similar interests share a house or unit and combine their separate funding packages to pay for support. Their combined funding is enough to pay for the 24-hour care that they require. This is used primarily by adults with disabilities.People can live in an established centre, such as a residential aged care facility designed to support the diverse needs of the clients.
