Glossary Client – a person receiving a service in their home. Worker – personal carer, care provider, nurse, social/welfare worker, therapist or other people performing health care or community work at the direction of an employer Primary carer -person who otherwise provides care to the client. Workplace – any place where work is, or is to be performed by a worker Care/case manager- Someone who provides individually planned and coordinated, flexible packages of care to provide for the complex care needs of older people Service delivery plan- A plan used by services to detail the use of funding assigned to support a person. Service delivery model in community setting People can live in their own home or a rented house/unit in the community. Care worker assist with the support they required. People can live in supported accommodation units. A few people with similar interests share a house or unit and combine their separate funding packages to pay for support. Their combined funding is enough to pay for the 24-hour care that they require. This is used primarily by adults with disabilities. Commonwealth Home Support Programme 1. Commonwealth Home Support Programme (CHSP) was introduced On 1 July 2015. 2. The CHSP consolidates the following four Commonwealth-funded home support programs into one streamlined and simplified program: Commonwealth Home and Community Care (HACC) program* National Respite for Carers Program (NRCP) Day Therapy Centres Program (DTC) Assistance with Care and Housing for the Aged Program (ACHA).
