Client #1 61 year old English speaking male admitted with fractured(fx) left femur, scheduled for surgery.  Pain rated 8 on scale of 0-10( 10 is worst pain).  Client has never been hospitalized with no past  medical history(pmh).  He is the father of three boys ages 8,4,2, happily married. Morning of  surgery entire family at bedside.  Upon entering room as his nurse client appears anxious,  asking many questions about the procedure and after surgery what to expect.  Client #2 83 year old Spanish/English speaking female admitted with dementia and dehydration. Lives in  nursing home frequently admitted because of poor fluid(po) intake.  Pt is hard of hearing in the  right ear and often fears health care providers.  Doctors have started IV fluids of D5.45 with 20  KCL at 125 cc/hour and would like nursing staff to encourage patient to drink.
