CIS 4004: Web Site Project – Phase 3 Page 2 © Dr. Mark Llewellyn For the final part of the website project, I’d like you to add a form to one of the pages of your website. It can either appear on an existing page, or you can create a special page just for the form. The form should as the visitor for basic information and you should use regular expressions in a PHP script to validate at least two fields that the user will supply. The action of your form should be to send you an email extracting the values from the form and including them in the email. See the lecture notes on forms for the technique.
