Mainly this occurs because we include a variable that is not experimental. If we are interested in studying the effect of a drug on a particular disease, then we can simply randomize. However, if we think the drug might affect males and females differently then we should use a block design. Gender will be the block and we will randomize levels of the drug within the male group and separately within the female group. When using variables like gender, a block design is the best option....
