HCR/220 Week five assignment Due May 29, 2011 1. Initial consultation is performed for a 78-year-old woman with unexplained weight loss, abdominal pain, and rectal bleeding. A comprehensive history and examination is performed. Initial consultation means new patient. A comprehensive history and examination having multiple symptoms would be code 99205. Rectal bleeding is usually a serious symptom so I believe it would be level 5. 2. A 30-year-old patient presents complaining of flu-like symptoms characterized by unremitting cough, sinus pain, and thick nasal discharge. An examination reveals bronchitis and sinus infection. The patient is prescribed a 5-day course of Zithromax. No history was needed from the patient so the patient is probably established. With the patients symptoms being confined to a particular part of the body it was a detailed examination with a code of 99214. 3. Established patient on Lithium presents for routine blood work to monitor therapeutic levels and kidney function. A nurse reviews the results and advises the patient that tests are normal, and
