Introduction The Registrar’s Office of any school or university is the office that is in-charge of taking good care of all the records of the present and past students of a certain university like the University of Santo Tomas, Manila. Our very own Registrar’s Office was currently headed by Mr. Cesar Velasco. In this office, the researchers chose the process of getting a Transcript of Records to be the center of the study. The researchers noticed the slow processing time of the papers that causes some problems to the customers which are present and former students of the university. The researchers brainstormed regarding the problem’s cause and effects. These causes and effects were compiled in an Ishikawa Diagram showed below (Figure 1). In this diagram, we can
