CHCMHS011 ASSESS & PROMOTE SOCIAL, EMOTIONAL & PHYSICAL WELLBEING Page 2 of 109 v 1.0 For this task, you are to work with two separate people ( these may be your friends or family members ) to collaboratively support their wellbeing: - You will do this by gathering information about their social, emotional and physical wellbeing, identifying the strengths and strategies that keeps the person strong - You will work with each person to identify other information to gather or people that support them to add to the understanding of their social and emotional wellbeing - You will discuss with the person their health and physical wellbeing and any health issues or potential risks to health including fitness and nutrition and encourage them to seek professional advice as require - You will discuss with them their cultural and spiritual wellbeing Use the following questionnaire for each person to gather information about their social, emotional and physical wellbeing.

I warm up and cool down by stretching before and after vigorous exercise. I feel good about the condition of my body. I get 7-8 hours of sleep each night. My immune system is strong and I am able to avoid most infectious diseases. My body heals itself quickly when I get sick or injured. I have lots of energy and can get through the day without being overly tired. I listen to my body; when there is something wrong, I seek professional advice. How do you rate your social wellbeing? I participate in a wide variety of social activities and enjoy being with people who are different than me.
