CHAPTER 65 ANXIETY Natural products: Kava: relaxant but causes hepatotoxicity 5-HTP and L-tryptophan: used for anxiety and depression, but associated with eosinophilia myalgia syndrome SJW: strong 3A4 inducer, photosensitivity, and serotonin syndrome Valerian: used for sleep, but may contain liver toxins Passionflower: may be safe and effective Drugs that cause anxiety: Albuterol (if used incorrectly) APs (aripiprazole or haloperidol) Bupropion Caffeine in high doses Decongestants Illicit drugs Levothyroxine Steroids Stimulants Theophylline SSRIs and SNRIs: escitalopram, fluoxetine, paroxetine, sertraline, duloxetine, and venlafaxine; First line Typically start at half dose for depression. Slowly titrate At least 4 weeks before you see an effect Buspirone: 7.5 mg PO BID; can inc by 5mg/day every 2-3 days,max dose is 30 mg PO BID Take with or without food, but be consistent May be used adjunct Better choice for augmentation than BZDs esp in elderly
