2THE DYNAMIC PROCESS OF CLINICAL JUDGMENTThe Dynamic Process of Clinical JudgmentThe first important patient scenario:The most urgent patient in the scenario presented is Mrs.EH. She was admitted due to acute onset of severe right upper quadrant pain, she has beenvomiting for the last 36 hours and her ECG monitor indicates Premature Ventricular Contractions(PVCs). This patient is the priority patient to assess based on her symptoms she is probablydehydrated due to severe vomiting, which can be related to the PVCs she is have because severevomiting can cause electrolyte imbalance and her pain level is stated to be at 10/10The second important patient scenario:The second patient to assess in the scenario present isMr. BW, this patient was admitted for fever of 103 and diagnosed with influenza. This finding isan indication of possible infiltrate in Mr. BW lower left lungs which might be an indication ofpneumonia which is related to the high temperature he is experiencing and it most likely due toan infection. The patient was reported to have rested well overnight and has been at the hospitallonger than the other patient which means his fever is probably under control and he is morestable compare to the first patient. He was diagnosed with influenza and his condition still needsto be manage and he will likely be placed on a scheduled antibiotic to help him get rid of theinfection but he is not the number one priority patient. The third important patient scenario:The third patient to consider in the scenario presented isMr. JR, he was admitted 8 hours prior for 24-hour hold to evaluate for possible myocardialinfarction. Serial troponins were reported to be within normal range, while the ECG levelsindicate evidence of anterior wall myocardial damage. He should be kept for observation but heisn’t top priority compared to other two cases
