[Type here] NR447 WEEK FIVE ASSIGNMENT Nursing Care Models Worksheet Student Name: Stephanie Mattessich Identify the model of nursing care that you observed. Be specific about what you observed, who was doing what, when, how and what led you to identify the particular model The model of nursing care I observed was the Interprofessional Practice Model. This model is a combination of multidisciplinary and interdisciplinary care. On my unit, every morning at 830 a.m. we start our patient specific rounds. These include one team member from every discipline and the primary nurse and doctor assigned to the patients. This takes place on a daily basis. Present at these interdisciplinary rounds are a doctor, a nurse, physical therapist, case manager, and a pharmacist. Together we discuss the patients’ needs in order for the best outcome. We review everything going on with the patient, if there were any acute changes, discharge plans, and the interventions needed in order to safely discharge the patient. Every member of the team gives their expertise on the patient and what they recommend. We discuss obstacles that could affect the patient’s outcome or discharge. The pharmacist reviews medications and discusses with the doctor and staff any changes that need to be made. The pharmacist also reviews medications for discharge, specifically availability and price. The pharmacist takes into consideration the patients’ socio-economic status in regard to medication needs after discharge. This is coordinated with case managers. The case manager discusses discharge planning. Physical therapy discusses if the patient is unable to return home due to mobility issues. Nursing discusses the overall care, medical issues as well as family or personal issues. We discuss if the patient is unable to dress themselves or walk, etc. We rely all the up to date information about the patient and their progress and make any recommendations accordingly. The doctor discusses mostly medical issues and then collaborates closely with case managers for discharge instructions and plans. On my unit, our case manager is a registered nurse. Basically, we are all there to discuss the patient as a whole. We hit every key point from medical issues, medication, discharge, family, etc. We collaborate together to come up with the best plan for the patient. Each member from a different health care discipline has something to bring to the table to help the patient. This situation led me to identify this as the interprofessional practice model because of the collaboration, communication, and integration of many disciplines in order to have the best outcome for the patient and their family. The team includes providers from different healthcare professions in order to meet the required patient needs (Finkelman, 2016).
