Foundations Lecture 1 9-7-10 (Week 1) Introduction to Cancer Medicine Learning Objectives Describe the characteristics of malignant cells: abnormal regulation of growth and survival, ability to invade and metastasize, genetic instability General Cancer Cell Characteristics - Make own growth signals (= growth ) - Ignore antigrowth signals o Generally will listen to own signals rather than outside signals - Evade apoptosis (no suicide) o As a cell continues to survive, it accumulates other mutations (real problem) - Keep growing indefinitely - Create feeder blood vessels - Invade normal tissue Benign Tumors - Growths that might push aside normal tissue, but do not dig into and invade normal tissue - Benign tumors cannot metastasize (movement to other places in the body) Differentiating with MALIGNANT Tumors – The Six Requirements for Deadly Cancer - Self-sufficiency growth signals 1

Lecture 1 9-7-10 (Week 1) - Insensitivity to anti-growth signals - major issue - Limitless replicative potential - Sustained angiogenesis - Evading apoptosis Why does cancer kill? - Invasion/destruction/mayhem in normal organs so that they can’t - Actions of the cancer disrupt homeostasis - Invasion and replacement of organs will predispose the victim to serious Describe essentials of cancer diagnosis: biopsy, histological and/or cytological examination of lesions, need to determine the type of cancer and the tissue origin Biopsy - Role of surgery: necessary for pathological exam to determine nature Radiologic Imaging - Usually done pre-op to see extent of tumor/resectability - Can also guide in biopsy - Aid in search for presence/absence of distant metastases Histological/Cytological Examination - Trumps radiologic imaging (radiologic imaging is not a histologic diagnosis; even though it may present as a tumor in the image, histologic evidence can show that it is not a tumor) Determination of type and origin - Along with epidemiologic studies, leads to discovery of the origin and how to potentially prevent and treat Correlate cancer incidence with geographic and environmental factors, age, heredity, and acquired pre-neoplastic disorders Why so much cancer? -
