Been a beside nurse for the past ten years and counting, I realize that interruption of care has a great impact in the healing process of the sick. In a home care setting, care coordination is critical because the focus of care is more holistic. Working into the home of a newly post hospitalized patient in need of infusion therapy, and wound care that has no willing caregiver will mean setting the stage for a delay healing process. As a nurse, I have to coordinate the patient’s care with the referring physician of all the findings in the home for immediate intervention to promote the patient’s healing process and decrease frequent hospitalization.
