Using deductive and inductive logic whenever possible; 1. Based on what you have seen, can you establish any causation? Largely this accident appears to be the result of pilot error. Poor CRM and poor decision making were both significant factors in this accident. The crew did not discuss the meteorological conditions or consider secondary or tertiary courses of action for the conditions. We did not get a lot of insight into the crew's mindset of physiological conditions before and during the flight so it is tough to say if fatigue may have had a role. The crew were carrying extra airspeed for the conditions and due to this had a tough time transitioning from flight to fully on the ground and stopping. The crew was also negligent when receiving the wind reports from ATC, at one point even reading them back incorrectly. At atleast two points there are wind directions and speed that would have exceeded the max cross/tailwind component for the 737-400. There is some
