Unformatted text preview: Click to edit Master subtitle style Assessment, Physical Exam Laboratory and Diagnostic Test in Endocrine System PHYSICAL ASSESSMENT Skin Assessment Inspect the color, LOOK for: Hyperpigmentation ( Addisons disease or Cushing Syndrome) Hypopigmentation (DM, hypo and hyperthyroidism) Yellowish cast on the skin (hypothyroidism) Purple striae over the abdomen and bruising (Cushing Syndrome) Hyperpigmentation Hypopigmentation Purple striae Palpate the skin, assessing the texture, moisture, and the presence of lesions Rough, dry skin in clients with hypothyroidism Smooth and flushed skin in hyperthyroidism Lesions on lower extremities that indicate Diabetes Mellitus Nail and Hair Assessment Assess texture and condition of the nails and hair, LOOK for: Increased pigmentation of the nails (Addisons Disease) Dry, thick, brittle nails and hair (Hypothyroidism) Thin, brittle nails and thin soft hair (Hyperthyroidism) Hirsutism (Cushing Syndrome) Nail Hyperpigmenation Dry, thick, brittle nails Dry, thick hair Hirsutism Facial Assessment Inspect the symmetry and form of the face Inspect position of the eyes Variations of form and structure may indicate growth anomalies such as acromegaly Exopthtalmos (Hyperthyroidism)- protruding eyeballs Facial Assessment Lid lag sign/ Von Graefe's sign (Hyperthyroidism)- is the immobility or lagging of the upper eyelid on downward rotation of the eye Dalyrimple s sign/ Thyroid Stare (Hyperthyroidism)- bright eyed stare-Infrequent blinking Facial Assessment Jeffreys sign (Hyperthyroidism)- forehead remains smooth when one looks up Thyroid Gland Assessment Palpate the thyroid gland for size and consistency. Stand behind the client, and place your fingers on either side of the trachea below the thyroid cartilage. Ask the client to tilt the head to the right. Now ask the client to swallow. As the client swallows, displace the left lobe while palpating the right lobe. Repeat to palpate the left lobe Thyroid Gland Assessment The thyroid may be enlarged in clients with Graves disease or goiter Multiple nodules may be seen in metabolic disorders whereas the presence of only one nodule may indicate a cyst or a benign or malignant tumor Motor Function Assessment...
