Assessment Data : Mr. A., age 52, was lying on his left side with side rails up. His vital signs were: Temperature 101.6 , pulse 76, respirations 20. Chest expansion was minimal, breaths sounds diminished on the left lower lobe. Has a productive cough of yellowish green mucus. Skin was warm and dry. Stated he was up most of the night coughing and sweated so much, he had to change his shirt. His wife was at the bedside reading a book. Had a BM yesterday after breakfast. He is on a regular diet and said he really likes apple juice.
