Unformatted text preview: SBAR Activity 0700 You have been assigned to care for Mrs. S in room 801. You read over her chart, kardex and MAR and note: She is 92 years old admitted with pneumonia yesterday. She is a patient of Dr. P. You obtained the following report from the off going nurse. Mrs. S had an uneventful night. VS stable with an O2 Saturation of 98%, breath sounds coarse, productive cough. Standard Precautions. 0730 You go to meet Mrs. S and complete your morning assessment. As you enter the room, Mrs. S. states I am so hot. You note that her skin is very warm and dry to touch. You assess her vital signs and they are: BP 120/80, HR 88, RR 22 and T 102.6 orally. You apply a cold wash cloth to Mrs. Ss forehead and go reassess the MAR for any orders for an antipyretic such as Tylenol. There are no such orders. You reassess the temp and it 102.8 now. 0745 You decide to notify the primary nurse and your instructor of the situation.
