Second and third generaton tyrosine kinase inhibitors Dasatnib and nilotnib are two alternatve oral second generaton TKIs that are also both licensed for use in newly diagnosed CP CML or as second line treatments after intolerance or failure of imatnib, and both appear to be more actve than imatnib. Currently, access to dasatnib in the UK is restricted to specific indicatons. More recently, two other TKIs, bosutnib and ponatnib, have also been licensed for patents refractory to other TKIs and show promising actvity in resistant cases. Some of these newer TKIs have, however, been associated with increased side effects and toxicity, such as the development of pleural effusions with dasatnib and an increased risk of cardiovascular and peripheral vascular events with nilotnib and ponatnib. The toxicity and tolerability profiles of the different drugs need to be monitored and borne in mind when choosing drugs for individual patents.
