Introduction and Background Information Name: Adriana Mejia Date of Assessment: 6/07/20 Reasons for Referral: Owing to Stacy's crying, weeping, and dropping down on the floor, leading to affection and escape, parents find it hard to control her at home and within the party. Target Behavior: Tantrum Operational Definition of Target Behavior: When Stacy cries (without being physically hurt) with and/or without tears, she screams or yells and/or falls to the ground while her arms and legs flail. Target Behavior onset: The target behavior starts as soon as it falls on the ground, crying, screaming, or yelling. Offset of Goal Behavior: When there is no crying or screaming for at least 10 seconds or when she gets off the ground Triggers to Tantrums: Nonpreferred Tasks/Activities When moving from preferred activities to non-preferred ones When parents tell her she can't carry her Teddy Bear to the party Community: When her parents ask her to get into the bath When parents request that she eat vegetables Reinforcers: Her Teddy Bear
