WEEK 5 QUESTIONNAIRE Week 5 Questionnaire Template For the Week 5 Application Assignment, you will interview two adolescents between the ages of 13 and 18 in order to gain insights about affective and social development during middle adolescence. Use this template as a guide to determine the questions you will ask during the two interviews. You may add or delete questions as you see fit, but be sure to cover all of the major topic areas identified below during the interviews. You will submit the completed questionnaires, including the questions you asked, notes summarizing the answers you received, and any other relevant observations, as part of this assignment. Identity o How would you describe your identity? How is your identity unique from those of your friends and family? 14-year-old girl: I am an honor roll student and on the freshman cheer team. I have a few friends that I like to hang out with after school and on the weekends. I think I’m nice and my friends tell me that I have the least temper out of all of us. My mom is very smart, but she is quiet and only has like one friend. I can be loud and cheerful. Sometimes we do not get along very well because she says I get too excited. My friends are a lot like me, but I think maybe I talk more! 16-year-old boy: I am a good student not all A’s or anything but good grades. I am a member of the marching and jazz band at school. I’m not really athletic at all. I turn in my work. I think I am a good son and brother. I have an afterschool job that I saved money from to get my own car. I try to help my parents at home by mowing and cleaning up after myself and my little brother. I even do my own laundry. I hope to go to college when I graduate. I want to take music and maybe teach the younger kids. I am a lot like my parents and even my little brother. We are quiet and tend to stay out of the way of other people. o What roles do race/ethnicity, spirituality/religion, and/or vocation/politics play in shaping your identity? 14-year-old girl: I have never really though about race. I mean I know I am white, but I don’t think that has anything to do with who I am as a person. My family isn’t the go-to church every single Sunday religious, but we do go to church and say grace. My mom is always telling us that lying, and cheating is sinful. I try to do right by God.
