TEAM EVALUATION YOUR NAME: _________________________ YOUR TEAM NAME: _______________________ This form provides you with the opportunity of ensuring equity and fairness in the grading of the teamwork in the class. It is possible that not everyone in each group deserves the same amount of credit for the teamwork done. Hence for the categories shown below, indicate the amount of work that each member of your team (including yourself !) contributed. Please use a 5-point scale: 5 points indicates that the individual took a lead role and made THE major contribution for that category. 3-4 points indicates that the person played a key supporting role. 2 points indicates that the person was active but took on a secondary role 0-1 point indicates that the person made a minor contribution or did not contribute at all to that part of the team’s efforts. NOTE: You cannot give every team member 5 points in each category. Please be honest and thoughtful in your scoring.
