HD 370 19:02 Psychodynamic Formulation, Cognitive therapy of Schizophrenia, Today’s Lecture 1. Psychodynamic Case: Liza (cont’d) with people suffering from Schizophrenia 3. Schema Therapy Universal Psychotherapy Components 1)Forming the Alliance Listening, making connections, demonstrating empathy, and also strength 2)Setting Goals Either explicitly or through wished-for versions of the future 3)   Creating an Interpersonal Space Putting the focus on the patience, the use of silence, speaking to the patient’s  emotional experience 4)  Interpreting what the patient does/doesn’t know Questioning, Clarifying, Confronting, Interpreting Schools of Psychology In Class Case: Liza B Initial Presentation    Upon admission Liza met criteria for major depressive episode, exhibited  psychomotor retardation and agitation, and stated continually that she did not feel  safe. During the first days on the unit she made an attempt to scratch herself and  was placed on one-to-one observation. Her formulation conference held by her multi-disciplinary treatment team  produced the following problem list: 1.  Suicidality  : consisting of ideation, impulsive gestures, and planned attempts

2.  Depression  : sleep disturbance, hopelessness, guilt, suicidal ideation, poor  concentration, anhedonia, fatigue, low self-esteem, irritability 3.  Self-Mutilating Behaviors  : includes scratching and cutting of wrists and legs 4.  PTSD symptoms  : flashbacks, nightmares, panic attacks associated with  memories of the sexual abuse 5.  Separation issues:   separation and perceived abandonments appear to  stimulate depression, PTSD symptoms, suicidality and self-mutilating behaviors 6.  Poor Lesiure Activities  : could list virtually no interests or activities aside from  talking with friends 7.  Mother/Daughter Issues  : turbulent dependence upon mom; rage at  separations, remarriage, and for the initial abuse by biological father
