PROJECT DESIGN AND METHOD2Chapter 3: Project Design and MethodThe present project aims to introduce the Agency for Healthcare Research and Quality (AHRQ) pressure ulcer prevention toolkits as a nursing intervention to reduce pressure ulcers at Porter Hospital. This chapter will provide a detailed description of how the DNP project director will actualize the project from the initial set up to completion and evaluation. The chapter is arranged into sections,of which the first three provide an organizational needs assessment and project stakeholders, followed by a SWOT analysis. This section will also consider the resources needed for the project the expected time plan, and the sustainability plans.Organizational NeedThe change agent has to examine both primary and secondary sources from the target hospital, department or population to determine the needs of an organization (Smolowitz et al., 2015). Secondary sources include poring through the hospital reports to assess the prevalence of pressure ulcers and determine most urgent needs. However, Cummings and Worley (2014) noted that primary research can be more effective in providing recent and up-to-date views on a problem. For the present research, the nurse director and administrator in the intensive care unit department at Porter's Hospital were interviewed. The interviews revealed that Porter Hospital was struggling with high rates of pressure ulcers in recent years and was at risk of falling into thebracket of hospitals that get penalties from Medicare for high rates (Byrne et al., 2016). The nurse director noted that the current policies and guidelines for pressure ulcers were developed years ago and had thus waned off in effectiveness due to changes in the hospital and staff turnover.
PROJECT DESIGN AND METHOD3Organizational SupportThe AHRQ toolkit's first section involves the assessment of the hospital's readiness for change (Berlowitz et al., 2014). Porter Hospital has seen an average number of quality improvement programs, whichhave been effective in enhancing the quality of care. However, there has been no recent program that proposes large-scale changes as the AHRQ toolkit for pressure ulcer prevention intervention. As Sterling (2015) noted, resistance to change is a significant barrier. However, the nursing director in the ICU was positive about the intervention and promised to support the project both financially and as the department's leader.
