1 PREO Update Package 2008-09-09 UPDATE TO THE PERMANENT REGISTER OF ELECTORS FOR ONTARIO To apply for an update to personal information in the Permanent Register of Electors for Ontario, such as a name change or a change of address, please complete the attached ‘ Application for an Update to the Permanent Register of Electors for Ontario ’ form. Submit the form with photocopies of proof of identity and proof of residence. Please see below for the list of documents that are considered acceptable as proof of identity and proof of residence. Please return the completed ‘ Update ’ package, including the ‘ Application for an Update to the Permanent Register of Electors for Ontario ’ form and photocopies of the required proof of identity and proof of residence documentation. 2 PREO Update Package 2008-09-09 Identification requirements for Update to the Permanent Register of Electors for Ontario To apply to be updated in the permanent register of electors, you must show proof of identity and proof of residence. Option 1: You may use one of the documents from List A to meet this requirement; or, Option 2: You must use one document from List B AND one document from List C . When applying for an update in person, only originals (or certified or notarized copies) will be accepted. Where the application does not need to be made in person, photocopies of identification will be accepted. List A: Identification shows Name, Address, and Signature An Ontario Driver’s Licence with your residential or 911 address A new Ontario Health Card (it is your choice whether or not you want to show this card) An Ontario Motor Vehicle Permit (the plate portion) with your residential address A cancelled personalised cheque with your residential address A signed mortgage, lease, rental agreement for your residential address
