June 9, 2010 To Athletes and Parents, The following information is extremely important for your athletic preparation and eligibility at Whitworth University. The primary concern of the athletic training staff and team physicians is to provide high quality medical care for you during your athletic career at Whitworth University. Please read these instructions, download the appropriate forms, and return them via email. There are a few changes to how we will be collecting information from athletes this year. The forms will be in a Microsoft Word format so each individual will be able to fill them out electronically. Please download the appropriate documents, fill them out, save them to your computer and email them back to [email protected] . If you do not have access to Microsoft Word and are unable to complete the forms, please contact Jon Bosh, assistant athletic trainer, or Melinda Larson, head athletic trainer, for a PDF version of the forms that can be printed and returned via hard copy. NCAA Educational Tools As part of providing high quality medical care, the Whitworth University Athletic Training Department wants to keep you informed regarding recent NCAA recommendations and educational efforts. The NCAA recommends confirming Sickle Cell Trait status in all student-athletes during the pre-season medical exam. Click here to learn more about sickle cell trait. Please make every effort to discover your Sickle Cell Trait status and confirm the test date on your medical history. Also, the NCAA recently mandated that all institutions have a concussion management plan on file. Whitworth University has had such a policy before and continues to update its policy in accordance with NCAA recommendations. Medical Forms 1) Health History NEW ATHLETES : Whitworth University’s athletic department policy states that all student-athletes must pass a preparticipation physical examination prior to participation in intercollegiate athletics. This initial examination will be provided free of charge on campus at pre-determined dates prior to the start of your season. Please see the schedule available on the website.
