June 9, 2010 To Athletes and Parents, The following information is extremely important for your athletic preparation and eligibility at Whitworth University. The primary concern of the athletic training staff and team physicians is to provide high quality medical care for you during your athletic career at Whitworth University. Please read these instructions, download the appropriate forms, and return them via email. There are a few changes to how we will be collecting information from athletes this year. The forms will be in a Microsoft Word format so each individual will be able to fill them out electronically. Please download the appropriate documents, fill them out, save them to your computer and email them back to athletictraining@whitworth.edu . If you do not have access to Microsoft Word and are unable to complete the forms, please contact Jon Bosh, assistant athletic trainer, or Melinda Larson, head athletic trainer, for a PDF version of the forms that can be printed and returned via hard copy.
