I N S T R U C T I O N S F O R C O M P L E T I N G T H E ( N A V M C 7 6 3 ) 1 Instructions for Completing the Appointment Acceptance and Record (NAVMC 763) he following instructions are to be used for the block-by-block completion of the NAVMC 763 rough that is submitted to MCRC with a complete Request for Appointment package. Dates All dates are recorded in 2-digit year, month, and day sequence separated by spaces, e.g., 23 January 1950 is written "50 01 23." Dates must not be entered on this form in any other format. Block 1. Name Record the name information as it appears on the Birth Certificate or U.S. Naturalization Certificate. If the individual was given an initial or initials rather than first and/or middle name, enter the initial or initials in quotations, i.e., "T," If the individual was not given a middle name or an initial, leave blank. Additionally, avoid unnecessary work by taking a moment to verify all your documents against the name on your source document. Block 2. SSN (Social Security Number) Enter the SSN in the three, two, and four digits format, i.e., "123 45 6789." Avoid unnecessary work; verify all your documents against the social security card. T

I N S T R U C T I O N S F O R C O M P L E T I N G T H E ( N A V M C 7 6 3 ) 2 Block 3. Date of Birth Enter the year, month, and day in a 2-digit sequence separated by spaces. Use the birth/naturalization certificate or passport as your source document. Compare this date on the other documents in your paperwork. Block 4. Home of Record This information can most readily be found on the most recent DD Form 4. Record the City and State information; disregard the County. The Home of Record (HOR) is the place recorded as the claimed residence upon appointment. Do not confuse HOR with state of legal residence for tax purposes. While some of the tests to establish each may be similar, these two items are separate. Under the Joint Federal Travel Regulations (JFTR), the HOR is defined as “the place recorded as the home of the individual when commissioned, appointed, enlisted, inducted, or ordered into the relevant tour of active duty.” The primary purp ose of having a HOR is “to determine the extent of the member‟s entitlement to travel and transportation allowances upon separation from service.” Upon appointment, the elected HOR will be established and it may not be changed unless there is a status change that would give the individual an opportunity to make a new election. Approval by CMC (MMSB-10) is required before correcting an erroneous HOR on the Appointment Acceptance and Record. Blocks 5 and 5A (Grade Appointed and Code)
