RETENTION INCENTIVE AGREEMENT This agreements is made and entered into on this ____ day of ___________, 2012, between Employee and Company. Employee and Company agree to the following: Employee was hired by Company on the _____ day of _______________, 2012. Employee was hired in the positon of _________________________________.
