TEST FORM > = TEST FORM MIS 111 - Fall 2006 Examination 2 Page 1 of 10 A Time allowed: 60 minutes Exam content: This exam covers all material covered in the lectures, readings, reports, and labs from September 28 th to October 19th, 2006. Exam value: 100 points (50 questions, each worth 2 points) Remember : This exam is closed book , closed notes , closed electronics (this includes laptops, cellular phones, PDAs, Blackberry’s, iPods or any other electronic device), or any other personal item, such as book bags or purses . You may NOT use a calculator or dictionary during this exam for any reason. After the exam begins, if you have to leave the classroom for any reason (bathroom break, emergency phone call, illness, etc), your must turn in your exam before you leave the classroom and you will not be allowed to continue your exam afterward. In you have comment or concerns of a general nature regarding the exam, please write your comments on the back of the Scantron form and contact your instructor after the exam via email. Instructions : On the left side of the Scantron form, complete the following sections: ID NUMBER (Student ID) and NAME. If your Student ID begins with an “S”, code the letter as a “0” (zero). In the “PHONE NUMBER” section enter the first six digits as “111 00x”, where “x” is your section number. You must mark all answers on your Scantron form with a #2 (HB soft lead) pencil. When correcting an error on your Scantron form, be sure that your incorrect answers are erased completely. Incorrectly marked questions (ones with multiple answers for the same question or mark that cannot be scanned) will be scored as incorrect.
