Informed Consent Form TITLE OF STUDY You are invited to participate in a small research project, being conducted as a course requirement, by _________________, who is a student taking a Social Methods course in the Sociology Department at Rutgers University. The purpose of this research project is to learn how to conduct a small survey (construct and administer a questionnaire, and analyze the results). I have chosen to ask the following research question for this course project: STATEMENT OF RESEARCH TOPIC. Approximately 30 subjects will participate in the study, and each individual's participation will last approximately five minutes. The study procedures include completion of a questionnaire, which contains a total of _NUMBER _ questions.

Unformatted text preview: If you agree to take part in the study, you will be assigned a random code number that will be used on each questionnaire. Data collection is anonymous. There will be no way to link your responses back to you. There are no foreseeable risks to participation in this study. Participation in this study is voluntary. You may choose not to participate, and you may choose not to answer any questions with which you are not comfortable.
