Consent Form for Participation in Comprehensive Fitness Testing Department of Health, Physical Education, and Recreation, Texas State University INTRODUCTION AND PURPOSE OF COMPREHENSIVE TESTING To assess your health-related physical fitness, including cardiorespiratory endurance, body composition, muscular  strength, muscular endurance, and flexibility, you will participate in comprehensive fitness testing. Your participation is  voluntary. However, understand that your current health status (as determined by your answers to a health history  questionnaire) dictates the degree to which you may participate in comprehensive testing. Read this form and ask  questions about anything you do not understand before signing this document. PROCEDURES After signing the consent form, you will be asked to complete a comprehensive health history questionnaire. Depending  on your answers to this questionnaire, you may participate in the following components of comprehensive testing: 1. Fill out a form about your health history;  2. Be measured for body weight, height, percent body fat with skinfold calipers, waist and hip circumferences, blood  pressure, and heart rate; 3. Be measured for aerobic fitness by riding a stationary bicycle, exercising on a treadmill, and/or stepping up and down  on a bench step; 4. Be measured for muscular fitness by a) performing curl-ups and push-ups, b) squeezing a hand grip dynamometer,  and c) performing hip-joint and shoulder flexibility tests.  POTENTIAL RISKS OR DISCOMFORTS
