SOAP note November 9, 2011/10:00 S. Pt was diagnosed with insulin dependent diabetes 5 years ago. He is struggling with adequate glucose control. He eats 3 meals a day with a morning, afternoon, and evening snack. He usually gives himself 3 injections a day . He usually eats breakfast at home, buys food for lunch at school, buys a snack on the way home (usually a big container of juice) with friends or eats a snack when he gets home, dinner is at home about 4 nights/week, evening snack is at home. O. 17 YOWM Ht: 70” Wt: 155# BMI: 22 kg/m 2 normal/average IBW: 166# (93% IBW) normal Labs: Blood sugars: before breakfast: 90 mg/dL okay Before dinner: 210 mg/dL high Before lunch: 150 mg/dL high Before bed: 110 mg/dL okay 24 hour recall: ~4300 kcal, 33 CHO exchange, 496 CHO g (46%), 11 PRO exchange, 144 PRO g (14%), 34 fat serving, 170 g fat (37%).
