Personalized Nutrition and Exercise Plan 2 Current health problems I am experiencing are related and can be managed through an appropriate diet and exercise program. High blood pressure and large body size are very influential in my family history. I was diagnosed at a young age with hypertension that results in high blood pressure. Activity and diet are key factors in maintaining high blood pressure but because it is a chronic disease, medication will be a necessity throughout my life. Struggling with overweight issues has been an ongoing problem in my life as well. Maintaining a good body mass index is a major component of regulating high blood pressure. Implementing a nutrition and exercise plan will provide a better opportunity to living a healthy and enjoyable life. Using the resources of MyPyramid Plan, establishing nutritional and exercise goals, and instituting a program that will provide action of obtaining the goals will create a successful outcome of my health needs. Anticipating obstacles and creating approaches to overcome them will be beneficial to the success of the health plan. Creating measures to follow the success and outcomes of the plan will be needed. Evidence of the nutrition and exercise plans effectiveness will be required to understanding the results of the high blood pressure and weight management needs. Evaluating and researching potential health risks will motivate the implementation and follow through of the plan. Addressing the never-ending changes of nutritional needs and activity throughout life will present a successful plan and quality of life. Nutritional and physical exercise goals will need to be regulated and changed throughout the plan to meet the appropriate needs for the current situation. The first nutritional goal is to establish and maintain a regulated diet meeting the recommended dietary allowances for my current age and body size. Reducing recommended dietary

Creating a daily menu to meet the appropriate needs of each food group and calorie intake will provide the proper action to meet this goal. Overcoming obstacles of a busy life will be managed by planning far enough ahead and preparing the menu for convenient availability.
