Debbie, One major obstacle I have witnessed repeatedly is peoples’ inability to identify there is a problem. Symptoms can easy interfere with our decision making abilities and deter us from solving the actually problem. Until recently my employer had a reputation for addressing symptoms but rarely fixing any problems. I often refer to this as the band- aid effect. The company spent so much time fixing all the symptoms but rarely cured the illness. I am glad to say that culture has changed. We have new policies in place that ensure all problems are investigated and solve to prevent future occurrences and delays....
