Surgical Case 4: Vernon Watkins Documentation Assignments 1. Document Vernon Watkins’ respiratory assessment that occurred in the case. Vernon had RR of 24 with chest movements equal during rise and fall. The respiratory system was assessed and lungs were clear and equal bilaterally and O2 was 96% on RA. Patient stated that it hurts to breathe and he did nothing before this happened. 2. Document the actions during the acute respiratory distress episode. Patient could not breathe adequately so HOB was elevated and V/s were taken and pulse oximeter was put on for continuous monitoring. I took the Vital signs and the provider was called and he entered more orders because possible PE was suspected.Because the pt O2 kept dropping I applied oxygen and started with 2L and then ended up increasing to 10L. The provider ordered 12 lead ECG, ABG, aPTT, Spiral CT, and CXR. 3. Document the changes in Vernon Watkins’ vital signs throughout the scenario.
