Niacin- B3 scovery – Pellagra: the 4 D’s – dermatitis , diarrhea , dementia & de Seen among poor peasants in Europe (Italy, France, Spain) and Egypt in the 17 th , 18 th and 19 th centuries. Began to be seen in US during the early 20 th century. Incorrectly linked to influx of immigrants as an infectious disease. related to disease Pellagra – in turn linked to corn Corn - a staple in the Aztec/North American Indian diet. Preparation: Aztec/Inca: soak in lime water or cook in ashes (alkaline). North American Indian: planted with beans and squash (three sisters). Pilgrims/pioneers copied Indians. But Conquistadors did not bring the practice of preparing corn with alkali back with them.

Discovery Continued High numbers of “pellagrins” in asylums. Hospitals would not accept and if the disease had progressed → dementia. Also saw higher numbers in the south due to poor southern diet. Three 3 M: m eat – pork fat back m eal - corn meal m olasses Dr. Joseph Goldberger : early public health physician , sent by US government • 1914 began work in the south – noted that staff that came in contact with inmates did not get sick. Thought diet was the problem. Theory dismissed by others. 1915 – controlled diet experiment in Rankin Prison, Jackson Mississippi demonstrated diet promoted pellegra – still not believed. 1916 – began meticulous public health surveys of cotton mill villages in the south

Surveyors picked up mild cases. 2) More common in people that did not have milk and meat in their diet – poverty. 3) Pellagra did not increase with the amount of corn, but rather what the corn displaced in their diet. 4)
