Abdominal Examination Pt. is supine and exposed from xiphoid process to below the ASISs. Pts. Arms are by their sides, and their head is on some form of support. *be sure to ask the patient if they have recently voided their bladder* Inspection I am inspecting for SKIN COLOR, MASSES, VASCULAR ABMORMALITIES, CONTOUR, AND OTHER ABNORMALITIES. Auscultation -I am listening to BOWEL SOUNDS ( use diaphragm ) for FREQUENCY AND CHARACTER *Listen to 3 spots (5 sec. each) in each quadrant (do them in order: LL, UL, UR, LR)* NORMAL RANGE IS 5-35 bowel sounds/minute 1-4 bowel sounds/minute is considered HYPOACTIVE More than 35 bowel sounds/minute is considered HYPERACTIVE COMPLETE ABSENCE of bowel sounds in 5 minutes is a MEDICAL EMERGENCY. -“I am listening for FRICTION RUBS at LIVER (right, ~T6) and SPLEEN” (Left, ~T8-T10) *use diaphragm * -“I am listening to the MAJOR ARTERIES FOR BRUITS” *use bell * AORTA (1 inch left, 1 inch superior of belly button)
