HEART EXAMINATION Pt. is supine, female in tube top (with hair clip/hair tie around material over sternum) * verbalize that this exam requires a tangential light source .* Inspection -I am watching for PULSATIONS AT THE AORTIC, PULMONIC, ERB’S POINT, TRICUSPID, MITRAL AND EPIGASTRIC AREAS. (A.P.E.T.M.E.) -I am looking for DYSPNEA, PRECORDIAL HEAVES, CYANOSIS, AND THE APICAL IMPULSE ( at mitral area) -I am also checking for ABNORMALITIES OF THE FINGERNAILS AND PITTING EDEMA at the ankles. Palpation “I am checking the (A.P.E.T.M.E.) areas for pulsations” (use finger pads) *while at the mitral area: “I am checking for the LOCATION AND AMPLITUDE OF An APICAL IMPULSE. *while checking for EPIGASTRIC PULSATION: “Pulsations coming from superior to inferior may indicate right ventricular enlargement .” “Pulsations coming from inferior to superior may indicate abdominal aortic aneurysm ." - I am now checking the (A.P.E.T.M.E.) areas for THRILLS (use ball of hand) Percussion “I will use percussion to IDENTIFY THE LOCATION AND SIZE OF THE HEART.”
