Blood Chemistry Testing Arterial Blood Gasses Electrolytes Metabolites Enzymes Carbohydrates Lipids Other Purpose of Testing Blood Gasses To evaluate the ability of the lungs to oxygenate the blood, and ventilate wastes Kidney function Ability of the blood to carry oxygen Arterial Blood Gasses pH = 7.35 to 7.45 PaCO2 = 35-45mm Hg PaO2 = 80-100mm Hg SaO2 = 94-100% HCO3- = 22-26 mEq/L O2CT = 15-23% Total Carbon Dioxide = 22-26 mEq/L pH = 7.35 to 7.45 pH measures the blood hydrogen ion concentration It is expressed as negative log 10 -2 is a pH of 2 10 -3 is a pH of 3 pH 2 is more acidic than pH 3 because there are 10x the number of hydrogen ions Blood must stay within a very tight range PaCO2 Measures the partial pressure of CO2 This is arterial blood so it should be low If this level elevates the blood is not ventilating properly This is the partial pressure of O2 in blood This level should be much higher in arterial (oxygenated) blood This measures the ability of the lung to oxygenate the blood SaO2 This is the oxygen saturation level of the blood It is expressed as a percentage of the carrying capacity of blood if all hemoglobin was saturated HCO3- The bicarbonate ion is crucial in maintenance of blood pH The concentration of bicarbonate is regulated by the kidneys O2CT Measures the actual amount of oxygen in blood Not often used in testing Abnormal Values Low PaO2 with O2CT and SaO2 with high PaCO2 Indicates not getting oxygen into the lungs either by muscle weakness, airway obstruction, or respiratory
