Lecture #18: The Context of Decision Making on the Floor Health Policy Example o Policy subsystem that cares about health policy finance is trying to control spending o Options: Cut back reimbursement to primary hospitals Result – people suffer from lack of local facility when they have a heart attack or stroke Cut back reimbursement to tertiary referral hospitals Result – Lower the quality of research and care Cut back the reimbursement to rural referral hospitals Lower quality medicine in those communities Normal Decisions o Not like the highly unusual votes, which are highly visible, have televised speeches, and are universally reported o Reported on inside of NY Times, and not at all in some newspapers o Few people outside the small group of professionals even care o Not party-line What does this illustrate? o These are complex issues, nothing like yes or no to paying for healthcare o Specific Not the creation of broad policy, but an incremental change
