Shock Types of shock Types of Shock Hypovolaemic Cardiogenic Mechanical Septic Anaphylactic Hypovolaemic shock Causes External blood loss Hidden blood loss – intra-thoracic, intra-abdominal, retroperitoneal, bony injury Other fluid losses such as from the GIT, urinary losses, cutaneous losses or losses into the tissues Pathophysiology Loss of circulating volume Reduced venous return and cardiac filling Reduced stroke volume – Starling’s law Reduced cardiac output ~SV x HR Reduced BP ~CO x SVR (systemic Vascular Resistance) Reduced oxygen delivery ~CO x [Hb] x SaO 2 Impaired cellular function Sympatho-adreno Response Reduced stretch of aortic and carotid baroreceptors, and blood flow to chemoreceptors causes reflex increase in sympathetic tone Heart – tachycardia and increased contractility Peripheral vessels – arteriolar and venous vasoconstriction
