Some say it the United States naval hospital  in1798. It had compulsory fees taken out to pay for  services. Others say it was the Dallas Teachers Association.  They negotiated a basic room, board, and E.R.  Plan with local hospital  1910 Montgomery Ward paid half wages for sick  days

ERISA ERISA 1938-1945 wages are frozen, so employers used  Health benefits to lure out the best candidates Many companies used self-funded plans to cut  costs by using taking money out of pay checks  then using left over for reinsurance to cover any  extraordinary costs  ERISA was created to make companies have to  disclose all benefits in writing
