Antibiotics Aminoglycosides Aminoglycosides inhibit protein synthesis by causing incorrect reading of mRNA Not absorbed orally so must be given iv Active against many GRAM -ve and some GRAM +ve bacteria. Narrow therapeutic window – increased risk of toxicity as accumulate in renal impairment. Damage CNVIII ----- Ototoxicity Therefore need regular blood levels Impair Neuromuscular transmission so contraindicated in MG Gentamicin Indications: severe infection – sepsis, meningitis, endocarditis. Also pyelonephritis/prostatitis, biliary tract infecions, pneumonia. Route/Dose: 5-7mg/kg ivi. Side effects: Ototoxic Nephrotoxic Rarely colitis Nausea and vomiting Seizures MG like syndrome. Caution : Obesity Breast feeding Pregnancy Renal or hepatic failure.

Increases effects of muscle relaxants and anticholinesterases. Strongly increases anticoagulation effects of warfarin . Tobramycin Streptomycin active against TB. But due to ototoxic effects if used in prolonged/intensive therapy has been largely replaced by rifampicin. Neomycin too toxic for parenteral use. Use topically in skin infections or orally to sterilise the bowel prior to surgery. Amikacin use in serious GRAM -ve infections that are resistant to gentamicin....
