Antibiotics Macrolides Macolides inhibit protein synthesis – bind to ribosomes Similar antimicrobial spectrum to benzylpenicillin. Narrow spectrum, mainly active against GRAM +ve, hence used as alternative in penicillin-sensitive patients. *Do not penetrate CNS so are ineffective in meningitis* Unlike penicillin are effective against several unusual organisms e.g. Mycoplasma and in Legionnaires disease Erythromycin Indications: atypical pneumonias, chlamydia, campylobacter. Penicillin allergy. Route/Dose: 500mg qds po (venous irritant) Side effects: GI upset (common) Dry itchy skin Hypersensitivity Arrhythmias Caution: Can increase QT interval Porphyria Breast feeding Pregnancy Renal or hepatic failure. Interactions :: Reduces P450 so increase ciclosporin, digoxin, theophyllines and carbamazepine.

Route/Dose: 250-500mg bd po Side effects: • As erythromycin but reduced GI upset Clindamycin Indications: osteomyelitis (good bone penetration), intra-abdomnal sepsis Route/Dose: 150-450mg qds po, 0.6-4.8g daily im/iv Side effects: • GI upset – colitis – stop if get diarrhoea • Hepatotoxicity • Blood disorders • Arthralgia • Hypersensitivity. Caution : • GI disease • Porphyria • Atopy • Breast feeding • Pregnancy • Renal or hepatic failure. Monitor : • U&E and LFTs. Interactions: • Increase effect of neuromuscular blocking agents....
