Quinolones Quinolones inhibit DNA synthesis Effective against GRAM -ve and atypicals (mycoplasma pneumoniae, chlamydia etc.) Ciprofloxacin Effective against most GRAM -ve bacteria and staph aureus. Indications: GI infections – salmonella, shigella, campylobacter. Resp infections – pseudomonas (not strep pneumoniae). Gonorrhoea. First line rx for anthrax. Route/Dose: 250-750mg bd po, 100-400mg bd ivi Side Effects: GI upset Neuropsychiatric effects (confusion, seizures, headache, hallucinations, mood changes), Tendinitis +/- rupture (esp. elderly taking steroids) Hypersensitivity (rash,fever etc) Avoid UV light and Fe/Zn containing products. Stop if – tendinitis, neuropsychiatric effects or hypersensitivity Contraindicated : Hypersensitivity to any quinolone Breast feeding or pregnancy. Caution : Can cause seizures MG – may worsen Interactions: Increases levels of theophyllines Increased risk of seizures if taken with NSAIDs
