Cardiovascular drugs ACE inhibitors ACE inhibitors prevent conversion of angiotensin I to angiotensin II, which is a powerful vasoconstrictor, this causes a fall in peripheral resistance and reduced BP . They reduce both preload (via venous dilation) and afterload (arteriolar dilation) so are useful in the treatment of heart failure They also do not impair cardiovascular reflexes and so do not have many of the side effects associated with beta blockers or diuretics. All taken orally od/bd Captopril short acting – largely replaced by longer acting drugs Indications: Hypertension, post MI, heart failure, diabetic nephropathy – persistant proteinuria Enalapril Indications: Hypertension, LVF Ramipril Indications: Hypertension, post MI, heart failure, CVS disease prophylaxis For all ACEI:

Unformatted text preview: Side effects: • Dry cough (due to increased bradykinin) • Rarely hypersensitivity - angioedema, proteinuria and neutropenia. • Sudden hypotension on first dose (esp patients on diuretics – due to hyponatraemia). • Renal failure in patients with bilateral renal artery stenosis. • Hyperkalaemia (due to reduced aldosterone secretion) if taking K+ supplements or on K+ sparing diuretics. Contraindications: • Renovascular disease – e.g. suspected bilateral RAS. • Pregnancy Caution: • Symptomatic aortic stenosis. • Renal failure. • Patient on other drugs that cause hyperkalaemia Monitor: • U&E for 2/52 after starting Interactions: • NSAIDs reduce effects • Diuretics increase risk of hypotension • Increases effects of lithium...
